最早的"First Lady"的称谓是由"first lady of honor", "first lady of the Admiralty"演化而来，用来这些称呼指那些有影响力的杰出 推荐信息女性。1834年更出现了"first lady in （of） the land"的称呼。爱德华-布尔沃-里顿在1851年引用了这一说法，"She was so beautiful and so good, and not proud she! Though she looked like the first lady in the land." "First Lady"第一次被用来指总统夫人是1849年,托卡里-泰勒总统称赞多利-麦迪逊夫人时说道，"She will never be forgotten because she was truly our First Lady for a half-century." （希拉里－克林顿在她的1999年多利-麦迪逊夫人纪念银币发行仪式上的讲话中引用了这句话。）
19世纪下半期，人们则很少用"First Lady"来指总统夫人，直到20世纪，"First Lady"才开始流行。1911年，埃尔西-费格森主演的名为"Dolly Madison" （又名"The First Lady of the Land"）的戏剧在纽约上演。1941年普林德维尔出版"First Lady"一书。《牛津英语词典》引用了《芝加哥论坛》的"Mrs. Thomas E. Dewey discloses today what kind of first lady she will be."
